Financial Literacy Workshop Registration Form

Applicants with net taxable income exceeding the following thresholds are required to pay a $10 fee per person:
• $45,000 per year for a 1‑person household
• $60,000 per year for a 2‑person household
• $75,000 per year for a 3‑person household
• $90,000 per year for a 4‑person household
• Households with income over $100,000 per year

Individuals with a taxable income of $0–$44,999.00 are eligible for FREE entry. However, they must still complete the registration form below to participate in the programs of The Monique Je' CEO Foundation free of charge.

Course will be held in the Community Room of Texas Republic Bank 2220 Sam Rayburn Hwy Melissa, TX 75454. Please arrive at least 5-10 early if possible to avoid missing any information or disturbing workshop by late entry.
