Providing Access and Transforming Health (PATH) empowers California's community-based organizations (CBOs), hospitals, and more to actively participate in the evolving Medi-Cal delivery system. With a dedicated $1.85 billion funding over five years, PATH addresses local capacity gaps, providing resources like personnel, billing systems, and data capabilities. This initiative aims to enhance community partners' ability to collaborate with managed care organizations, expanding services for Medi-Cal beneficiaries. PATH is committed to promoting health equity by addressing social determinants of health, working towards a coordinated and accessible Medi-Cal system meeting the diverse needs of Californians.
