The model includes a range of tools, such as the CNRA, an online caregiver management system, personalized intervention and monitoring templates, and detailed operational guidelines. By rigorously evaluating the CSM’s impact on caregiver support, this study seeks to advance the development of evidence-based strategies in informal care, addressing the complex needs of caregivers of frail older adults in Hong Kong and across the world.
