What does 77% of patient AR have in common? When we analyzed patient-owed dollars across the typical health system, we found that they fall into two categories at the highest level: 23% are relatively easy to collect from — digitally engaged patients with solid insurance coverage. The other 77%? Harder to collect. And growing fast. A portion of that AR comes from large bills patients simply can't afford. But an even bigger driver is underinsurance. These are patients who technically have coverage, yet their deductibles and out-of-pocket costs exceed what they can realistically pay. The problem? Most collection approaches rely on either credit-based propensity scores or broad-stroked segmentation buckets. Both miss critical context. A patient who paid a $20 copay last month could still be labeled "high propensity" — even after losing coverage this month and receiving a four-figure bill.
