Comments
A. This week I arrived at clinical
twenty minutes before report was
given. In that time I was able to get
everything I needed for the day. In
that time I was able to look up some
of my patients info on their charts.
B. This week a nurse delegated a task
to me. I was charting and a nurse
came up to me and asked if I was
busy. I stated no, and she asked me
If I could help a patient to the
restroom. I went and helped him.
C. This week I was assigned three patients. I
was able to provide organized care to each
one of my patients by performing daily
assessments, passing their medications on
time, doing hourly rounds, and answering
my patients call lights. My assignment was
also a primary assignment, meaning I had to
be more organized than normal because my
nurse and I were the only ones giving care to
our patients.
D. I was able to adapt to clinical situations
this week when working with different
patient populations. One of my patients was
fully independent while the other was
dependent. I was able to adjust and take care
of each patient safely and effectively.
E. This week I was able to perform my
morning assessments before 9oclock. I also
was able to chart them in the moment as
well. My nurse was a primary nurse this
week one of the days and was in charge of
doing all of her patients care. I was able to
document and chart accurately and timely on
vitals and assessments so she wouldnt have
as much work to do. I also kept on task with
charting the hourly rounds, and making sure
my patients were content and didnt need
anything.
F. I was able to keep on top of everything
this week by following my very organized
nurse. She taught me time management skill
and organizational skills. I was able to keep
on top of all my patients wants and needs by
doing hourly rounds, and asking them if they
needed anything before they called out for it.
Doing hourly rounds is important because it
lets the patient know you care, and are
organized in your health care.
G. A system theory is when a variety of
people work together to achieve a goal. This
week I watched a group of people work
together to help get my patient discharge.
They advocated for my patient, and helped
get his insurance to cover the medications he
needed.
H. During clinical I worked with my nurse,
my patients nurse tech, and my patients
doctor, as well as my patients diabetic
counselor. Making sure each
interdisciplinary team is working together on
the same tasks and goals, make the patients
stay in the hospital more effective and
shorter.

Comments
A. One of my patients was having bad
issues related to pain. She was given Norco
and dilaudid yet she stated that the pain
medications didnt even touch her pain
located in her left hip. I was able to
reposition her and give her heat packs which
she stated helped her tremendously.
Sometimes a comforting technique is all that
is needed to help a patient with pain.
B. Plan of care for one of my patients were
to have him stay in the hospital until is
Coumadin increased his INR. He had been in
the hospital for a week waiting for his INR
to increase and was ready to leave. We were
able to get everyone on board and agree that
he would be sent home with lovenox to act
as a bridge until we were able to increase his
cumadin to a level that will increase his INR.
He was happy with that plan.
C. My pts nursing diagnosis was acute pain
related to infection in the hand. Evidenced
based research stated that we would give the
patient pain medications to decrease the
pain, as well as use comforting techniques to
decrease the patients pain. Also to get rid of
the infection in the patients body that is
creating the pain in the first place.
D. This week I did not see any of my
patients family members or hear them talk
about any issues related to their families.
E. This week I was able to communicate
effectively with my nurse, nurse tech, and
classmates.

Comments
A. During the morning med pass one of my
patients just needed a nicotine patch while
the other patient needed a blood pressure
medication as well as was calling out for
pain medication. I used clinical reasoning
when choosing the patient I wanted to give
meds to first. I chose the patient who was
calling out for pain medications as well as
the one who needed blood pressure
medications.
B. My patients hand was hot to touch,
reddened, had discharge coming out of
wounds, as well as was painful. I was able to
assess the hand and determine that the
patient had an infection before even reading
her chart on why she was at the hospital.
C. When taking my patients vitals I noticed
his blood pressure had increased
tremendously from doing them in the
morning. I was able to distinguish that and
tried takin his BP on the other arm. The other
arm gave a better number that was closer to
the data that I had earlier.
D.My patient was diabetic and required
insulin. My nurse and I checked his BS then
we corrected his mealtime insulin and
calculated how many units we would give
the patient. Once we determined the units we
had another nurse double check it. Then I
administered the insulin by cleaning the area
with an alcohol swab, letting it dry, giving
the insulin, holding it for 10seconds after I
gave the insulin.
E. On Sunday my patient was told his
insurance wouldnt cover the lovenox shots
that he needed. This created him to have to
stay in the hospital longer until his INR labs
increased. This was a problem to the patient
because he wanted to get out as soon as
possible because he had life and work to get
back to. Working with other
interdisciplinary teams as well as insurance
company we found a plan that would get him
his lovenox shots covered which would let
him get discharged from the hospital.

Comments
A. I worked well with my nurse, doctors on
the floor, and nurse techs who were on the
floor. I was also able to also work well with
my classmates by helping them when they
asked for help.
B. This week I was courteous and honest
with all my patients. I was honest with them
all by telling them what I was going to do
before I did it.
C. This week I was able to irrigate, clean,
change a dressing, and flush my first drain
tube. When doing all of these tasks my nurse
walked me through it and provided
constructive criticism to what I was
performing.
D. I was accountable for all my actions
during my clinical experience.
E. This week I was able to take care of a
patient who was of a different culture and
religion than me. I didnt form any bias
towards him and performed care on him like
I would any other patient.
