There was an error in the registration of the said vehicle, as the


body type purportedly is a VAN when in truth and in fact, the
appearance and the actual body type is that of a JITNEY.
This affidavit is executed to attest to the truth of the above facts
for the purpose of CHANGE OF BODY TYPE and for whatever legal
purpose this may serve.
IN WITNESS WHEREOF, we have hereunto set our hands this
