StarHub Credit Card Payment Application

I, hereby instruct StarHub Ltd to debit my credit card (as stated in Part 2) in payment for all Charges* billed to the StarHub Account(s)
as stated in Part 1. This recurrent credit card payment arrangement/ authorisation will continue to be in effect until the StarHub
Customer(s) for account(s) stated under Part 1 notify StarHub in writing to terminate it, or StarHub receive a notification from the credit
card company. StarHub reserve the right to terminate these recurrent arrangements at any point in time at their discretion.
*Charges means all amounts due and owing to StarHub, including but not limited to amounts in StarHub bills, penalty fees, purchase
charges and any other ad hoc charges as may be imposed by StarHub. I agree to StarHub Ltd collecting, using and disclosing my
personal data for the purposes of processing this recurring payment arrangement, payments and refunds.

Note :
1. The information given in this application will supercede all relevant information we have in our systems.
2. For new application, this authorisation will take effect from customer's next billing cycle.
3. If we are unable to make the deduction or settlement with your card company for any reason whatsoever, StarHub customer(s) shall remain liable
to StarHub for the charges under StarHub Customer Account Number(s) above. Further, we will not be liable to you for any charges incurred on the
credit card as a result of our deductions as authorised herein.
4. StarHub Customer(as in Part 1) will see in their bill "Total outstanding balance will be deducted from your Credit Card account <no.> on <due
date>" once Credit Card arrangement is in effect.
5. In the event that the StarHub bill is paid by a consenting third party, StarHub shall take written instructions from the StarHub customer (as stated in
Part 1), including but not limited to payment instructions and services subscribed to.
6. StarHub refers to StarHub Ltd and / or the relevant Affiliate providing the particular service, as set out in the StarHub Consumer Terms &
Conditions.
