A chest x-ray was performed on Baby Girl Pesado, born prematurely on 1/15/2013, which revealed bilateral pneumonitis. Pneumonitis appears as streaky infiltrates in the inner lung zones and can occur in premature infants as their lungs are not fully developed. The heart, great vessels, diaphragm and other chest structures appeared normal in size and configuration. As a preterm infant, Baby Girl Pesado is at risk for lung complications like pneumonitis due to developmental immaturity. Her condition will be monitored and treated with oxygen therapy and any medications prescribed.

Interpretation
Streaky infiltrates are seen in both inner lung zones
Heart and great vessels are normal in size and configuration
Diaphragm and sulci are intact
The rest of the visualized chest structures are unremarkable
Impression: PNEUMONITIS, BILATERAL
Description:
According to Adelle Pillitteri, Chest Radiography shows areas of infiltration or
consolidation in the lungs; if a foreign body is opaque, an x-ray study will show its location. In
addition to radiography, it can furnish an accurate picture of the heart size and contour and size of
the heart chambers. It can reveal fluid collecting in the lungs or pulmonary artery from cardiac
failure. It also can be used to confirm the placement of pacemaker leads. In a posteroanterior view,
if the cardiac width is more than half the chest width, it indicates that the heart is unusually
enlarged.
Chest Radiography is helpful in detecting problems to the premature infant because there
are several complications that can occur in that stage such as pneumonitis and respiratory distress
syndrome and this can be detected and treated right away in the help of chest x-ray.
Procedure:
Usually chest X-rays are taken with the body in an elongated position. Generally, a premature baby
will be placed on a table and gently held, arms up, by the nurse. The X-ray machine is turned on
for a fraction of a second. Beams of X-rays pass through the chest to make a picture on the film.
Usually an X-ray is taken from the front and then the side. The procedure is painless and the
photographic results should be ready within minutes.

Nursing Responsibility:

Assess patients pre, intra, and post procedures.

Educate the family about the procedure and post care.

Administer medications, including moderate sedation, as ordered by physician and


monitor patient outcomes.

Analysis:
According Behrman RE, Butler AS 2007, developmental immaturity affects a wide range
of organ systems reason why there are complications on premature infants. It describes the shortterm complications of preterm birth in terms of fetal development as well as injury to fragile organ
systems during the perinatal and neonatal periods.and one of those is pneumonitis.
As seen on physicians interpretation above, Baby girl chest structure are all normal aside
from her lungs which she was diagnosed to have pneumonitis bilaterally where the inner lungs
streaky infiltrates, meaning there are abnormal substance that is accumulated in both lungs. It is
possible to the pre term infant to have this kind of complication because on this period the infants
lung are not yet mature due to its preterm delivery and this can be treated by monitoring the patient
and giving her a warm and moist oxygen therapy and administer medication that will prescribed by
the physician.
