availableSunThur9AM6PM(localtime).ProvideavalidtelephonenumberandemailaddresswhenschedulingsoPrometriccan
emailyouaConfirmationNoticeandcontactyouintheeventthatyourappointmentmustberescheduled.TheConfirmationNotice
containsaPrometricConfirmationNumberYouneedthisnumbertoconfirmorchangeyourappointment.
2. CONFIRMORCHANGEAPPOINTMENT:ConfirmyourappointmentusingyourPrometricConfirmationNumberoneweek
beforeyourappointmentatwww.prometric.com/USMLEorbycallingthetelephonenumberabove.Youmaychange(e.g.,
reschedule,cancel,changetestcenterlocation)yourappointmentwithintheeligibilityperiodnotedabove.Toavoidanappointment
changefee,youmustchangeyourappointment31ormoredaysbefore(butnotincluding)thefirstdayofyourscheduledtest
date.AlldeadlinesarecalculatedusinglocaltimeinLelystad,Netherlands.Ifyourescheduleyourappointmentorchangeyourtest

3. ELIGIBILITYPERIODEXTENSION:Ifyouareunabletotestwithinyoureligibilityperiod,contactECFMGtoinquireabouta
onetime,contiguousthreemontheligibilityperiodextension.Afeeischargedforthisservice.VisitECFMG'swebsiteformore
information.Ifyoudonottakethetestwithinyouroriginalorextendedeligibilityperiodandwishtotakeitinthefuture,youmust
reapplybysubmittinganewapplicationandfee(s).
4. PRACTICESESSION:TotakeaPracticeSessionataPrometricTestCenterforafeepayabletoPrometric,registerontheUSMLE
website,www.usmle.org,usingtheScheduling#andCINfromthispermit.YouwillbeissuedaPracticeSessionSchedulingPermit
withinsevenbusinessdaysofregistering.Uponreceiptofyourpermit,youmaycontactPrometrictopaythefeeandschedulean
appointment.
5. ADMISSIONTOTHETESTCENTER:YoumustbringthisSchedulingPermitandanunexpired,governmentissuedformofID
(suchasadriver'slicenseorpassport)thatincludesbothyourrecentphotographandsignature.ThenameonyourIDmustmatchthe
nameonthispermitexactly.Theonlyacceptabledifferencesarevariationsincapitalizationthepresenceofamiddlename,middle
initialorsuffixononedocumentanditsabsenceontheotherorthepresenceofamiddlenameononeandmiddleinitialontheother.
ThenameonyourIDmustappearintheLatinalphabet,i.e.,in"Englishlanguageletters."IfthenamesonyourIDandpermitdiffer,
contactECFMGimmediately.Youcannotmakeanamechangeorcorrectionwithinsevenbusinessdaysofyourscheduledtest
date.IfyouarerequiredtocarryaPermanentResidencyCard,besuretobringitwithyoualongwiththeotherrequiredformofIDas
notedabove.
6. TESTDAY:
ARRIVAL:Reporttothetestcenter30minutesbeforeyourscheduledappointment.Youmustsigninandyourphotographwillbe
taken.Ifyouarrivelaterthanyourscheduledappointment,youmaynotbeadmitted.Ifyouarrivemorethan30minutesafteryour
scheduledappointment,youwillnotbeadmittedtothetestingcenter.
PERSONALITEMSTORAGE:Youmustplaceallofyourpersonalitems,includingcellphones,pagers,watches,wallets,
food,andbeveragesinasmalllocker.Pagersandcellphonesmustbeturnedoff.
TESTING:TheTestCenterAdministrator(TCA)willinstructyoutowriteyournameandCINlistedaboveonalaminatedwriting
surfaceprovidedformakingnotes.YouwillenteryourCINintothecomputeratyourworkstationtoinitiateyourexamandtoresume
testingafterbreaks.DonoteraseyournameandCINfromthelaminatedwritingsurfaceuntiltheendofyourtestday.
7. ENDOFTESTDAY:TheTCAwillprovideyouwithaprintoutthatconfirmsyouappearedforthatday'stestingsession.Onceyou
leavethetestcenter,Prometriccannotreproducetheprintout.
