3 MMDA, DPWH, DENR-ECC, CAAP (ATO), DOH-BHDT
4 QC Subdivision Admininistration Office (if townhouses or subdivisions)

5 Bldg. Permit &/or Certificate of Occupancy of all existing buildings within


the subject lot and one (1) complete set of approved building plans, if
application is renovation / addition or extension
6 Contractor's / City Tax (pre-requisite for fast track applications)

- BP 344 (Accessibility Law) compliance, if applicable
