with special order number (SO), date of graduation and Seal of the School.

Original Certification of transcript of records stating non availability of such equipment to


be signed by President or Registrar for school who do not have scanning equipment for
pictures.

Birth Certificate (NSO authenticated)

Marriage certificate for married female applicants

Four (4) passport size ID pictures with name tag

Community Tax Certificate (Cedula)

One (1) piece metered stamp window mailing envelop

Certificate of units earned for non BSA graduates only

NBI clearance
