AFFIDAVIT OF LOSS

I, EDWIN JESUS O. BUENDIA JR., of legal age, Filipino, married and a


resident of B12 L34 Phase 2 Wonderland Homes Subdivision, Sta. Teresa, Iguig,
Cagayan, after being sworn to in accordance with law, do hereby depose and state:

1. That I am the holder of a Philippine Passport which was issued to me by the


Department of Foreign Affairs, Tuguegarao City and which has already
expired;

2. That sometime on last month, I discovered that my aforementioned passport


which I placed inside our cabinet in our house was lost and/or missing and
despite diligent search, I failed to recover the same;

3. That in case the lost passport have been found, it will be rendered null and
void and of no effect whatsoever;

4. That I am executing this affidavit to attest to the fact of loss of my


aforementioned passport and for further purpose of securing another from
the Department of Foreign Affairs, Tuguegarao City in lieu of the lost one.

IN WITNESS WHEREOF, I have hereunto set my hand this 2nd day of February
2018 at Tuguegarao City.
