What you'll need

To complete this application form you will need digital copies of your documents as
you will be prompted to upload evidence at section 4 of the application. Digital copies
can be scanned copies or photographs etc. of your documentation; please note we can
only accept pdf and jpeg files

Please ensure you have all of the following information and documents before you
start your application, as failure to upload the supporting documents as instructed will
result in your application being declined as an "incomplete application".

 Evidence of your nationality and identity (passport, identity card, driving


licence etc.).

 Evidence of your change of name if different to that on your qualifications or


identity.

 Details of your qualifications.

 Details of your employment within the last ten years if you are applying based
on your employment record.

 Copies of your qualifications.

 Certified translations of you qualifications.

 Copies of your transcripts or Diploma Supplements.

 Certified translations of your transcripts or Diploma supplements if they are


not bilingual.

 Evidence of additional training you may have undertaken to become a fully


qualified school teacher in an EEA country e.g. evidence of completing
induction or a practice year.

Please note we do not need certified translations of your identity or change of name
documents.
