1. Intelligence Officer 1 Marco G. Puno provided an affidavit stating that he photographed a drug buy-bust operation that resulted in the arrest of Hubert A. Bautista. 2. Puno photographed the briefing, marking of evidence, inventory at the police station witnessed by representatives of the barangay and Department of Justice. 3. AFFIDAVIT OF PHOTOGRAPHER

I, INTELLIGENCE OFFICER 1 [IO1] MARCO G. PUNO,


of Legal age, Filipino and a bonafide member of the Philippine Drug
Enforcement Agency (PDEA) and presently assigned at PDEA VIII
located at PDEA ROVIII with an office address at Brgy. Candahug,
Palo, Leyte after having been duly sworn to in accordance with law do
hereby depose and state:

1. That I am a member team composing operatives of our


Office, Palo PNP (SDEU) and Leyte Provincial Drug
Enforcement Unit (Leyte PDEU) that conducted Buy-
bust Operation at Brgy. Sampaguita, Tacloban City, Leyte
at about 3:10 pm of June 29, 2018 which resulted to the
arrest of HUBERT A. BAUTISTA, 43 years old,
Filipino, single, self-employed and a resident of Brgy. Lily
Justice Road, Tacloban City, Leyte;

2. That I was the designated PHOTOGRAPHER of the said


anti-illegal drugs buy-bust operation against HUBERT
A. BAUTISTA;

3. That I have used S7 Edge Samsung cellular phone with


camera in taking photographs;

4. That I took photographs during the final briefing of our


designated team leader for the conduct of an anti-illegal
drugs buy-bust operation against our suspect/subject;

5. That I also took photographs during the markings of the


purchased/seized drug and non-drug evidence from the
suspect said HUBERT A. BAUTISTA at the place of
transaction and arrest at Brgy. Sampaguita, Tacloban
City, Leyte;

6. That I likewise took photographs during the conduct of


Physical Inventory of the purchased/seized drug and non-
drug evidences held at Tacloban City Police Station as in
the witnessed by the elected Hon. Julius V. Morita
(Brgy. Kagawad) of Brgy. Sampaguita, Tacloban City,
Leyte and Representative from Department of Justice
(DOJ) Roger H. Diaz of Provincial Prosecutor Office,
Tacloban City;

7. That I finally took mugshot photographs of the arrested


suspect, HUBERT A. BAUTISTA at Tacloban City
Police Station, Tacloban City, Leyte;

8. That anything not mentioned in this affidavit will be


narrated during proper trial;

9. That I executed this affidavit to attest the truthfulness of


the above narrated facts and to support our charge for
violation of Sections 5 and 11, Article II of R.A. 9165
against the above named suspect.

FURTHER AFFIANT SAYETH NAUGHT:

IN WITNESS WHEREOF, I have hereunto affixed my signature


this 30th day of June 2018 at Tacloban City Police Station, Tacloban
City, Leyte, Philippines.
