TO WHOM IT MAY CONCERN:

This is to certify that MARVIN UPHOLSTERY owned and operated


by (DELA CRUZ, MARVIN SAMSON) with business address located at
(BARANGAY TINANG CONCEPCION TARLAC) after complying with the
requirements prescribed by this Office, is hereby issued a
BARANGAY "CLEARANCE OF BUSINESS" for the purpose of securing a
permit in order to operate a (UPHOLSTERY BUSINESS) in the said
area.

In accordance with the Barangay Revenue and Ordinance of


2008. However, the Barangay Permit clearances may be
cancelled or revoked at anytime if pulic safety and
interest may required. This is to certify further that the
herein subject is the Owner of ( MARVIN UPHOLSTERY ) located in
this barangay.

This certification is issued upon verbal request of


the herein subject for whatever legal purpose it may serve.
