The undersigned law firm requires an acceptance fee of THOUSAND PESOS

(Php,000.00). In addition, ten percent (10%) of the recovered amount shall form part of the
contingency (success) fee of the undersigned firm.

Apart from said acceptance fee, the undersigned law firm requires that a deposit of at
least THOUSAND PESOS (Php,000.00) be remitted to start working with the conduct of the
witness interview and will cover until the case reaches Trial (minor motions, affidavits of witnesses
and additional necessary transportation fees has yet to be determined and included and will be
subject to the rates below).

In case the matter will be settled amicably, the following rates shall apply:

1. Settlement Agreement Php,000.00


2. Other incidental papers, ,000.00 per paper
not constituting a major
paper
3. Appearance fee ,000.00 per appearance
4. Transportation Allowance ,000.00 per trip

Should a criminal case proceed in court, the following rates shall apply:

1. Major pleadings Php,000.00 per pleading


2. Motions, affidavits, ,000.00 per paper
and other incidental
papers (except Motion
for Reconsideration)
3. Appearance fee ,000.00 per appearance
4. Transportation Allowance ,000.00 per trip
