Dear Governor Juanito Victor C. Remulla Jr.,

My name is Ana Carmela B. Domingo. I am a citizen of Paliparan III, City of Dasmariñas, Cavite. I am
currently enrolled in Cavite State University - Imus Campus and pursuing Bachelor of Science in Business
Management Major in Human Resource Management. I am applying for educational assistance in this
Provincial Scholarship Program.

During Senior High School, I was awarded an academic award with honor. The course that I am pursuing
is aligned to the track that I took in Senior High. I have interest and passion regarding business.

Since I am a second year college student now, this educational assistance would be a great help to cover
my expenses such as transportion, food fees and other expenses related to my study. This would also
help me to study well without worrying and getting stress to cover up my financial needs.

I am presenting my application form with a copy of my previous semester grades for your approval, as
well as my registration form to verify my perseverance in my studies.
