SUBJECT: ORCA Taguig Security Guard and Facility Personnel Incident

Dear OIC Alcala,

In line with the incident happened last _______ involving BAT Security Guard Renato Calpito with ORCA
employee, Facility Supervisor _______________, this is to document that the ORCA Management is giving
SG Calpito a go signal to resume his work and report back with ORCA Taguig on __________.

After careful assessment and discussion, ORCA Operations has decided to return SG Calpito on his duty but
with commitment and assurance that there should no longer incident of dispute, or even an attempt while he is
within the premises and on-duty. ORCA is practicing a Good Operational Standards that applies from its
Employees down to its Support employee system. Failure to do so will result to rebuke and final disposition of
the ORCA management to SG Calpito. Having that said, we are also requesting SG Calpito a written
commitment statement on this matter.

Prior to reporting back to work, we are inviting you and SG Calpito together with __________ and his
Manager to sit down with us and close this issue smoothly.
