1. Susie is satisfactory in identifying word frazes.

2. Susie is adequate in addition and subtraction problems.

Areas of Need

3. Susie needs improvement in reading comprehension.

4. Susie needs help with multiplaction.

Instructional Accommodations:

Writing Skills

5. Allow more time for written tasks including note taking, copying and tests for Susie.

6. Susie will be given handouts from the teacher during the writing lesson.

Arithmetic

7. Teacher will list and color code steps and procedures for Susie.

8. Teacher will provide frequent checks for accuracy while Susie is doing classwork.

Word Problems

9. Teacher will incorporate smart board, or tablet for visual learning for Susie.
Reading Comprehension

10. Provide Susie with a study guide of test before each test.

11. Allow Susie to use sticky notes and highlighters to mark important information.

Cursive Writing

22. By Annual review date of the 2019-2019 school year, in a classroom setting, Susie
will write legibly using writing in upper and lower case on her papers, achieving criteria
of 90% accuracy ,measured by observation, documentation, and teacher-made tests, as
implemented by the Special Education Teacher, General Education Teacher, and
Teaching Staff.

Arithmetic Benchmarks:

23. By Annual review date of the 2019-2020 school year, in a classroom setting, Susie
will practice multiplication facts achieving criteria of 70% accuracy, measured by
observation, documentation, and teacher-made tests, as implemented by the Special
Education Teacher, General Education Teacher, and Teaching Staff.

Word Problems Benchmarks:

24. By Annual review date of the 2019-2020 school year, in a classroom setting, Susie
will learn rounding to the nearest whole number achieving criteria of 90% accuracy,
measured by observation, documentation, and teacher-made tests, as implemented by
the Special Education Teacher, General Education Teacher, and Teaching Staff.

Reading Comprehension Benchmarks:

25. By Annual review date of the 2019-2020 school year, in a classroom setting, Susie
will recall the five W's after reading, achieving criteria of 90% accuracy, measured by
observation, documentation, and teacher-made tests, as implemented by the Special
Education, General Education Teacher, and Teaching Staff.

Cognitive Learning Skills Benchmarks:

26. By Annual review date of the 2019-2020 school year, in a classroom setting, Susie
will be allowed to use study guides while taking tests achieving criteria of 90% accuracy,
measured by observation, documentation, and teacher-made tests, as implemented by
the Special Education Teacher, General Education Teacher, and Teaching Staff.

Social Skills Benchmarks:

27. By Annual review date of the 2019-2020 school year, in a classroom setting, Susie
will participate in non-preferred discussions achieving criteria of 90% accuracy,
measured by observation, documentation, and teacher-made tests, as implemented by
the Special Education Teacher, General Education Teacher, and Teaching Staff.

Implementing and Monitoring

28. Mr. Jones will work with Susie once a week for 2 hours beginning February 1st,
2020. Susie will be in a low stress environment and will be working on her
communication skills in a group setting in the Resource Room.

29. Susie will be able to tell her parents everything she did with appropriate verbal skills.
30. Susie will be able to share new learned concepts with her parents.
