NOTE:

1. This request form should be sent along with a photocopy/scanned copy of the Degree
Certificate of the candidate.
2. Without receiving verification fee, verification process shall not be carried out.
3. Central Govt. / State Govt. / Autonomous organizations / PSUs etc. claiming for waiver
of payment of verification fee are requested to submit documents/self-declaration where
it should befwefwefwcdsdcsdcsdce mentioned that their organization is empowered
through certain O.M. for fee waiver.
