Subject: Absent Without Leave/ AWOL

Date: March 03, 2019

The matter of your prolonged absence has been brought to our attention. In particular, it

appears that you failed to report for work on March 11, 2019 to March 15, 2019.

Records show that your absence was not authorized under approved leave application. In this

regard, please be informed that unauthorized absence or absences without leave (AWOL)

constitutes gross habitual neglect of duties, which is a ground for termination under the Labor

Code. On record, this is one of several instances of absences without leave.

In view of the foregoing, you are hereby required to submit a written explanation to this office

within (5) days from receipt of this notice, stating why the company should not terminate your

employment.

Your failure to submit your written explanation within the period provided shall be construed as

a waiver of your right to be heard and the Company shall decide the matter on the basis of all

records and evidence available to it which may lead to your separation from the company.

Your compliance is strongly encouraged.
