ASSESSMENT TASK 2-SCENARIO

Scenario 1
Ellen is a 45 year old woman who lives in supported accommodation following a workplace accident that left her
with physical disabilities and an acquired brain injury. Your role today is to assist Ellen to get ready to attend a
doctor’s appointment and then to go to lunch with a friend. Ellen is able to transfer from bed to chair and into her
wheelchair using grab rails and bars. She uses a wheelchair when she goes out of the house, as she has little strength
in her legs and cannot walk without leaning very heavily on supports. Her acquired brain injury has left her with
some speech difficulties and word finding difficulties. She also has difficulty in taking in large chunks of information
all at once. In order to help her get ready, you are to supervise her transfer from bed to wheelchair to shower chair.
While she showers independently, you are to prepare her chair and towels. Once she has completed showering, you
are to assist her to choose appropriate clothing, help her to get dry, dressed and groomed and, finally, assist her to
prepare her breakfast.
a) Write a script (100 words) showing how you would confirm the procedures you are going to use in providing
support to Ellen.
I will use simple vocabulary and non-verbal communication strategies to confirm the procedures. “Good morning,
Ellen” with a smile on my face I will greet her. Then I will ask her politely “are you feeling good to engage in the
process of transferring from bed to wheel chair and then to shower chair”. I will give her some time to process what I
said. After her permission I will supervise the same one by one and not in rush. When she will be showering on her
own I will prepare her chair and towels. I will ask her “are you done with shower?” I will show her the clothes to
choose from and let her decide. When she will have chosen the clothing I will seek her approval for nonverbal or
verbal response. After that I will help her to dry herself and getting ready if she needs by confirming from her. Then I
will help her in making breakfast by maintaining her independence. When she will be done having her breakfast I will
ask for any further assistance and let her be comfortable in the wheelchair to doctor’s appointment.

b) Write down three (3) ways you could encourage Ellen to actively participate in her care and identify her
preferences where possible.
 She can shower herself and can be encouraged to choose the clothing.
 She can also tell what type of hairstyle she wants to make her more presentable.
 She can also actively participate in the breakfast making and can choose what to eat.

c) Ellen asks you whether you know of a way she can get into a car or taxi without getting out of her wheelchair as
she is finding this quite exhausting.
a. Research to find information about how a person in a wheelchair can get into a car while staying in their chair.

b. Write down the steps you would take in providing this information.
 Firstly I will research online about the related information and also ask for the assistance from the supervisor
too.
 After gathering all the information I will appropriately try to describe it to the client.
 Once she has approved I will do the further assistance required.

Scenario 2
Simon Paxton has returned home from hospital after a motorcycle accident in which his partner was killed. His
injuries have left him a paraplegic, with no voluntary control of his bladder. He has depression as a result of the
accident. Simon lives alone in a ground floor apartment.

A) Identify the type of personal care support might Simon require? Outline some of the requirements you expect
might be central to Simon's personal care plan.
Simon requires long term support care with targeted services and multidisciplinary care. His care plan may include:
 personal hygiene, including showering, bathing, oral hygiene, dressing and grooming;
 toileting, bladder and bowel management;
 eating and drinking;
 attending appointments;
 mobility and transferring, for example moving in and out of bed and on or off the toilet;
 Application of splints, basic first aid due to injuries sustained as a result of his disability.
 Support when undertaking social, recreational, education or employment activities
 the medical condition of the participant, including any medication required;

b) Name three (3) types of aids, equipment or processes that Simon or a support worker assisting him might
require to complete his personal care tasks and activities of daily living.
 Mobility aids
 Modified equipments for eating.
 Aids for showering
 Hoist and mechanical lifting

c) A backup support worker will be required to assist Simon with his shower. Develop a checklist to make sure that
the work is performed to the required standard with respect to:
● Use of equipment to assist Simon:
1. Hoist to transfer from bed to wheel chair and then showering chair.
2. Once he is transferred to shower care worker will assist him with shower according to his will. As he can
clean himself well but he will require someone to help him.
3. He can choose to be assisted by a male support worker if he wants.

1. Risk of injury while transferring to shower chair.
2. Vulnerability due to no privacy.

Scenario 3
Mary is the support worker providing care support for Mrs Meadows in her home. Mary helps Mrs Meadows to
shower, dress, do her hair and make-up, and to be ready to get on the bus that picks her up for the day centre.
Today when she is drying Mrs Meadows, Mary notices a large bruise on her back and another smaller one on her
arm. When Mary asks Mrs Meadows how they got there, Mrs Meadows tells Mary she tripped on the path to her
clothesline and fell. She says this happened yesterday and she still feels quite stiff and sore. Once Mrs Meadows is
dry and dressed, Mary rings her supervisor to report Mrs Meadows’s injuries. The supervisor, Margaret, says she will
contact Mrs Meadows doctor and also let the day centre coordinator know. Margaret asks Mary to go and look at

the path to see what Mrs Meadows tripped over. Mary looks and sees that part of the path is cracked and has lifted.
She can see large tree roots growing under the path. She returns to the phone and describes this to her supervisor.
