Greetings!

The University is now facing great challenges in the provision of quality


education in this time of pandemic. A series of trainings for faculty
enhancement to meet the demand of online classes was conducted in order to
continue to produce quality graduates. The usual didactic and experiential
teaching and learning process is now compromised particularly to the Allied
Health Programs offered by the University.

With this, the undersigned would like to take this opportunity to apply for a
limited face to face classes for Allied Health programs subject to compliance
with the Joint Memorandum Circular of the Commission on Higher Education
(CHED) and the Department of Health (DOH) with checklist provided for the
safety of the students and personnel of the campus in this time of pandemic.

Your favorable response to this application is our privilege to help our students
cope with their losses in the actual hands -on laboratory experience and the
critical skills training of students in the Related Learning Experience (RLE).

Attached are the prepared documents in compliance with the checklist


provided by your office.
