[ ] [ ] Structural Analysis and Design as a whole structure based on the existing
old structural design
[ ] [ ] Duly signed and sealed Certification of a Structural Engineer , accredited by
ASEP, certifying that the existing structures is capable to withstand the
additional floor/storey
[ ] [ ] Structural Engineer structural review
III. For old structure, if necessary
[ ] [ ] Evaluation report with structural test conducted such as hammer test, etc.
[ ] [ ] Amended Structural Analysis & design of whole structure incorporating
the result of the said structural evaluation
[ ] [ ] Duly signed and sealed Certification of a Civil Engineer
[ ] [ ] More than 30% slope
