Summary from a video titled “Process Safety: Who’s Responsible?”

In this video, Nick, one of the workers in oil and gas industry, has explained or shared
about his experience in the industry relating to the importance of understanding and carrying
out all procedures and regulations that apply, especially in terms of process safety. He wants
to explain that things because he has experienced a terrible incident in his workplace.

The safety process is not just following the regulations and procedures but is putting
many of protectors such as barriers or malwares to maintaining safety and our products from
various risks that can occur because all processes controlled by humans, which sometimes
there will be accident occurred caused by human error.

He also explains an example of an explosion that occurred in the "Yut Energy" of a


leaked hydrocarbon gas, due to a corrosion pipe where Nick was one of the survivors of the
accident. The incident started from many of engineer has being moved to another plant and
bugets cuts made by the company.

One month after cutting funds and reducing workers, the industry got huge profits,
full capacity production and everything rans smoothly, but engineering managers think of
reducing workers and adding tasks to workers it should not be done and even enginers think
the same but because of pressure from superiors they can only follow what has been ordered.

Day after day passed, by reducing personnel and adding tasks to workers, make some
reports to be done and here began the negligence due to personnel reductions such as
corrosion reports on some instruments that were not reviewed directly and there were no
detailed reports. The reports only based on the signatures stating that everything is in good
condition, at this stage the "slippage failure procedure" occurs. Nothing can be done just by
signature without looking the really condition of the instruments in the field.

In the final stage, there is a "stop working & validation failure" where the instruments
has run into a severe corrosion stage which causes the hydrocarbon gas to be released freely,
negligence of managers and workers who are not concerned with the details of the report.
Like time bomb, finally the explosion happens that killed workers and not only losses but
also sadness for people left by loved ones.

The conclusion of this video is that whoever we are, when we are going to work in the
industrial world, it is very important for everyone to understand and not to ignore the Safety
Process so that undesirable things will not happen.
