This is to certify that Mr. Sreejith S.V, 25 years is under treatment as

out Patient in my Vaidhyasalai Since 25-12-2016 for Dengue Fever. He has

recovered from his illness but the body condition is not fit to resume duties.

So he is advised to take rest for two months from 20/1/2017 onwards for

the restoration of his health.
