List of Requirements:

• Birth Certificate (PSA or NSO)


• Original NBI Clearance o If you need to create an
appointment for your NBI, here’s the link:

• 2 Valid IDs (any of the following) o Passport o
Driver's license
o Professional Regulation Commission (PRC) ID o Postal ID o Voter’s ID
o Taxpayer Identification Number (TIN) o Government Service
Insurance System (GSIS) e-Card o Social Security System (SSS) card o
Philhealth ID o Pag-IBIG Loyalty Card o NBI Clearance
• SSS Employment History o Visit SSS Online:
o Under the Member Info link, click Employment History. This will open
your Employment History page.
o You can now view your Employment History page. Here you will have
access to your SSS employment history report with information such
as the name of your employers, their employer ID, reporting date,
and employment date. Please take a screenshot and send the copy to
us.

NOTE: If you still do not have your statutory numbers/government number (ex. First employment).
Please make sure to register on the links below. You’ll be required to submit them before you start your
training.

 BIR/TIN: For first time applicants, prepare 1 original copy of your PSA Birth Certificate. The
company will start the application for you.

Please make sure to complete your Pre-Employment Medical Examination and finish the Background
