_______5. Joint Venture Agreement, if Applicable
_______6. Special Power of Authority to be signed by the registered owner
_______7. Site Development Plan with complete attachments
_______8. Affidavit of Undertaking of stating that they will commence the development within 1 year upon approval
_______9. *Certification from Department of Environment and Natural Resources (DENR), if property fully planted
with fruit-bearing trees.
_______10. *Certification from Philippine Coconut Authority (PCA), if property if planted with coconut
_______11. *Certification from National Irrigation Administration (NIA) if property is rice land,
_______12. *Environment Compliance Certificate, if available
_______13. Certificate of No-Coverage issued by the Provincial Agrarian Reform Office, Lipa City
_______14. Evaluation Report signed by Engr. Marlino A. Babida, Asst. MPDC
_______15. Zoning Certification to be issued by EnP Monalisa P. Robledo MPDC
_______16. Certification issued by the Municipal Agrarian Reform Office that property is not covered by Land
Reform Law and that there is no tenancy issue involved
_______17. Certification from Agriculture Office signed by MAO, Felix E. Leopango, Municipal Agriculture Office
_______18. BARC Clearance of BARC Chairman of Brgy where property is located
_______19. Kapasyahan ng Sangguniang Barangay stating that they agree with the proposed reclassification
_______20. Barangay Clearance/Business Permit issued by Brgy. Captain where property is located
_______21. Barangay Certification that subject property is located in the said barangay
_______22. Real Property Tax Clearance of subject property Municipal Treasurer’s Office for the year 2021
_______23. Vicinity Map and Tax Mapping Control issued by the Assessor’s Office
_______24. Valid ID of applicant

IMPORTANT NOTICE TO ALL APPLICANTS:
Only reclassification fee to be computed at Php18.00 per square meter and application fee of Php 5,000.00 payable to
the Office of the Municipal Treasurer shall be collected from all applicants. No other fees shall be assessed.Coordination
and follow-up only with the Office of the SB Secretary Jun. Perez.
