located at # 18 Maamo St. Cor. Anonas Ext. Sikatuna Village, Quezon city authorizes MR.

CHRISTOPHER CASABAL to be my representative to process in my behalf to your

representative to process in your office for the Certified True Copy of our Business Permit

mentioned above that will use for business retirement due of bankruptcy.

In view thereof, I will pay the City Government thru him all fees, penalties, and the like that are

issued with official receipt only.
