1. 請將填妥的表格及「體能活動適應能力問卷與你」連同上述證書／證件副本一併交回康樂及文化事務署各體育館辦理登記
手續。申請人在登記時須出示身分證明文件（正本或副本），以供查核。
Please return the completed form together with a copy of the above mentioned certificate(s) / document(s) and “Physical Activities Readiness
Questionnaire-Par-Q & You” to any sports centre under the Leisure and Cultural Services Department for registration. At the time of
registration, applicants must produce their identity documents (either the original or the copy) for verification.

2. 處理申請一般需時約一星期，視乎個別情況而定。
It normally takes about one week to process the application depending on individual circumstances.
3. 申請人提供的資料只供申請登記使用康樂及文化事務署健身室及日後聯絡之用， 本署授權人員基於上述目的方可查閱。
申請人如欲更改或索取所申報的個人資料，請與接受申請的體育館職員聯絡。申請人如未能提供所需的個人資料，本署
可能無法處理有關申請。
The information provided by the applicant will be used for the application for registration as a user of fitness rooms under the Leisure and
Cultural Services Department and for future contact purpose only. Only persons authorised by this Department will have access to such
information for the aforesaid purposes. If applicant wishes to amend or retrieve the personal information provided, please contact the staff of
the sports centre who has accepted the application. If applicant fails to provide the required personal information, this Department may not be
able to process the application.
