Clinical Instructor: Valery Cruz - Lim RN. Community CAPO

LEARNING FEEDBACK DIARY

Objectives:
• To be able to learn how to consider the language of others.
• To be able to conduct a thorough assessment of the family's client.
• To be able to learn how to teach health teaching efficiently and successfully.

My first day of community duty was physically demanding because the roads and distance
between the barangay and the river make it challenging to reach their community. But it is worth
the effort. I learned how to listen to others' complaints and assess the family's concerns. I learned
how to socialize with others. Apart from that, I can see the true essence of why nursing is a calling.
Even though it is difficult to visit their community, we do so to provide proper health education
and hope to at least lessen their illness.

It is also heartbreaking to see and hear about other people's financial problems, many people are
unemployed, but they are still willing to accept a non-relative into their home and accept them as
a true family. On my first day, I asked myself, "Why am I so ungrateful for things when many
people have a hard day every day and still manage to fight for their everyday life?" Aside from
being physically demanding, it is also emotionally draining. As a result, I conclude that money
cannot buy happiness as long as you are healthy and have faith in God to fight in everyday life.
Today is the day. A lot of realizations come to my heart, and this is the best learning I've received
from madam Valery Lim and the people in this community.

LEARNING FEEDBACK DIARY

Objectives:
• to be able to create a Nursing Care Plan
• To be able to learn how to organize Flyers to raise patient awareness.

My second day's duty was to have fun! because we did not visit the community. We still
manage to make the most of our time. We made flyers to give out on our third day of duty, and
by doing so, we learned how goiter affects people and what the client needs to overcome this
disease. We also created an NCP to make a concrete plan for how we are going to help or teach
the client about health.

LEARNING FEEDBACK DIARY

Objectives:
• To be able to help the family harness their human materials resources
• To be able to understand the process of action- reflection - action
• To be able to know the needs of the individual, the family during health and illness.

My third day of duty under madam Valery Lim was great! After assessing the patient on day
one and creating flyers on day two, we did health education with the client on day three, and we
can now proceed with our intervention to the client in our nursing care plan. Following that, we
demonstrate how to do hemoglobin and why it is necessary to always check the protein in our
red blood cells. Now I know why I occasionally experience orthostatic hypotension, and I can at
least prevent it by taking vitamins with extra folate or eating vegetables, fruits, meat, and
chicken. Before leaving, we planned our last day's activities, which included an open clinic, a
feeding program, health education, and Zumba. This day was fantastic!

LEARNING FEEDBACK DIARY

Objectives:
• To help people understand their own situations and develop awareness
• To promote health and efficiency
• To be able to provide health services, preventive care, intervention and health promotion
to individual, families and community.

Today is the day. I learned why nursing is a calling because when you love what you do, you
don't care how far or how expensive it is because your goal is to make them happy while also
reducing or preventing their illnesses.

This is the final day in the community setting under Madam Valery Lim. Since day one, I have
had a superficial patience due to the environment and weather, but this last day has left me
wanting to return to the community setting duty because the people in barangay Capo are very
courteous, and our clinical instructor is both kind and strict, but you will definitely learn from
her. Because of the open clinic, I learned how to master my CBG and hemoglobin skills,
particularly how to establish rapport with patients in order to gain their trust and allow me to
prick their finger. Aside from that, because of the feeding program and health education it helps
me how to socialize and be confident, I've realized that even though it takes a lot of effort and
money to implement this kind of event into the community and provide care to individuals,
families, and communities, you won't think about it or notice it because it is your passion. It's
worth it to see and hear them say thank you. You will become less tired. Now I understand why
nursing is a calling: you will care for those who do not care for themselves.
