MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

The Balantoy Elementary School of the Department of Education with office address at Balantoy, Balbalan,
Kalinga represented by JOCELYN D. MAGOLLADO, School Principal, hereinafter referred as to the “FIRST
PARTY”.
and
Barangay Health Workers represented by the Barangay Captain, ALBINO D. BALAO-AS, of Balantoy,
Balbalan, hereinafter referred to as the “SECOND PARTY”;
WITNESSETH THAT:
WHEREAS; the Balantoy Elementary School of the Department of Education’s thrust is to ensure access and
quality basic education for all Filipino learners through effective implementation of the programs, activities and
projects;
WHEREAS; the Balantoy Elementary School is applying for Limited Face to Face to ensure the effective
implementation of the programs, activities and projects;
WHEREAS; the second party will commit themselves to cooperate with Balantoy Elementary School
personnel in the implementation of health protocols to ensure the safety of all learners, personnel and visitors in
the school comes limited face to face classes.
NOW THEREFORE, for and consideration of the foregoing premises and stipulations herein below set forth,
the parties have agreed and hereby agree and shall take effect upon the signing of the parties and shall be valid
and binding until the completion of its purpose or unless terminated by the parties.
WITNESS WHEREOF, the parties hereunto set their hands this 4 th day of April 2022 at Balantoy Elementary
School, Balbalan, Kalinga.
