1. PURPOSE

Transporters should treat Pilot as an asset to the company. A first-class Pilot will result to a high
efficiency of delivery in a long run to the company. This includes lower maintenance and repair to
vehicle, not to mention lowered down losses due to zero accident cases, pilferage cases and Lost
Time Injuries (“LTI”). The Pilot is the heart of the fleet organization which may reflect to behavioral
change.

2. SCOPE

This Module provides the procedures and tools required to recruit a new Pilot into PETRONAS
Chemicals Marketing (Labuan) Ltd. (“PCML”) System. The Module includes:

The requirements stated in this module will take precedence if contradictory to PETRONAS Technical
Service (“PTS”).

3. PERFORMANCE MEASUREMENTS

Ensure that employees working in key positions meet minimum experience and training
qualifications and that personnel performance is regularly assessed for competency.

4. PROCEDURE

4.1 Process Flow

Below is the procedure and processes of new Pilot recruitment as a guideline for Contractor
(Transporter). Pilot need to undergo below step by step process to become a PETRONAS Chemicals
Group (“PCG”) approved Pilot.

4.2.1.1 Working Experience


Applicant need to provide all the information regarding his previous job before signing off.
Transporters are requested to contact the previous employer to verify the information given and to
obtain more information regarding the applicant. This information will be included together in the
Application Form in 3.1. Transporters are requested to file a copy of this form for future reference.

4.2.2 Recruitment Criteria Checklist


The selection is recommended to follow criteria below. The essential part is must have item whereas
the desirable is an advantage to have.

4.2.2.2 PCG Pilot’s Unauthorized List

Pilots who have been unauthorized by PCG are not allowed to carry PCG cargo load. Transporters
shall write/e-mail to PCG to determine whether the Pilot is listed under PCG unauthorized list, either
permanently or temporarily.

4.2.2.3 E-Vetting Status

Pilots need to be vet for criminal records by PETRONAS Group Security Department (“CSD”) and
Kementerian Dalam Negeri (“KDN”) prior to entering any PETRONAS premises to conduct work
related activity. Transporter shall submit the application via e-Vetting Online System at
evetting.cgso.gov.my.

Pilots will be issued with ‘verification card’ if passed the screening and need to carry the card
whenever entering PETRONAS premises. Further details on the submission can be find from the
website. The typical process is as per Appendix 3.7.

4.2.2.4 Numeracy and Literacy Test

Transporters need to gauge level of numeracy and literacy competency of the applicant to avoid
any miscommunication when conducting their daily job. It is recommended, Transporters to use
the form to get the result. The applicant may repeat the test, depending on the Transporter. For
actual test questions, please refer to Appendix 3.4.

The test paper contains 20 questions.


• 17 objective questions – Based on Road Traffic Rules & Regulations
• 2 subjective questions – Calculation based
• 1 subjective questions – Opinion based

4.2.2.5 Parking Test

Transporters need to gauge the applicant can demonstrate basic vehicle control to ensure Pilot can
safely maneuver/park as part of their daily job. This test shall be conducted by Transporter’s Driver
Trainer. Transporter need to prepare the layout for the test accordingly. For actual form, please
refer to Appendix 3.5.

4.2.2.6 Practical Driving Test

Transporters need to gauge applicant’s level of knowledge to drive by Transporter’s Driver trainer
before proceeding with the recruitment. This is to see the level of readiness to become a
professional Pilot. Applicant will fail the test if he gets overall score below 80%. The applicant may
repeat the test, depending on the Transporter.

4.2.3 Medical Checklist

Transporters have to ensure that medical check-up are being conducted at PETRONAS Approved
Medical Examiners (AMEs) panel clinic, or OSH certified medical examiners. Transporters are
requested to file a copy of this form for future reference. However, this does not absolve
Transporter responsibilities to conduct medical check-up as per regulatory requirements

4.2.3.1 Frequency of Assessment

i. Pre- entry
ii. Every 2 years basis. The frequency may be increased if clinically indicated.
iii. After significant or devastating illness, or change in medical status, including, but not
limited to the following conditions:
a) Cardiovascular-coronary artery disease, angina, arrhythmias, congestive heart
failure, severe hypertension, stroke
b) Endocrinology- diabetes mellitus
c) Neurologic- epilepsy, narcolepsy, nervous system tumors, syncopy, vertigo
d) Visual Impairments
iv. Following a road transport accident where health related condition such as illnesses and
substance misuse are suspected to be a potential contributory factor to the accident, OR
returning from lost workday case accident of more than 4 days. Assessment should be
arranged as soon as possible to ensure accuracy of the potential findings.

4.2.3.2 Content of Assessment

i. Questionnaire and Declaration & Consent Statement


ii. Physical examination as appropriate, depending on medical history, but always including
the following:
a) vision-visual fields, acuity, color, adequacy of correction from corrective lenses (if
appropriate)
b) hearing-audiometric testing or equivalent
c) physical signs associated with sleep disorders
d) cardiovascular-stethoscopic examination
e) blood pressure
f) clinical neurological examination
g) muscular-skeletal system

h) electrocardiogram for all above 40 age group


i) cardiovascular risk assessment for all above 40 age group using Framingham Risk
Score. Individuals assessed to be at high risk score (10 year risk of cardiovascular
event of > 20%) are required to be further evaluated by a Cardiologist.
j) measurement of average sleep propensity or ‘daytime sleepiness’ using Epworth
Sleepiness Scale (ESS) questionnaire. Questionnaire and details of the usage is
available for clinical use at official website of EES

iii. Laboratory evaluation as appropriate, depending on history or physical exam findings,
but at least including:
a) Fasting Blood Sugar
b) Full Blood Count
c) Serum Lipid Profile (include detail of cholesterol count)

Drug and Alcohol screening as required by Company Policy. The D&A screening
may be conducted as part of the medical exam or separately.

4.2.3.3 Interpretation of the result

i. Consider exclusion if there are significant clinical findings of:


a) Presence of medical conditions that may cause sudden incapacitation (coronary
heart disease, valvular heart disease, pneumothorax, uncontrolled hypertension,
uncontrolled diabetes, stroke).
b) Musculoskeletal and neurological problems that may affect physical movement
and coordination (Parkinsonism, severe migraine, muscular dystrophy, paralysis,
epilepsy etc.). Presence of physical handicaps that render a person unable to
safely operate the vehicle, e.g. amputated limbs and severe achondroplasia.
c) Visual acuity worse than 6/12 for both eyes despite using corrective lenses. Depth
perception and visual field shall be normal. Mono-ocular vision is acceptable if
depth perception is not affected. Individual who require lenses, should carry a
spare pair of glasses while at work.
d) Severe colour blindness. Those with mild colour perception deficiency, trade test
may be required.
e) Average hearing loss of more than 35dBA for hearing frequencies of 0.5 – 2 kHz
in the good ear.

f) BMI of more than 35. Lower BMI with associated complications such as
uncontrolled Hypertension, Diabetes, etc. may render the individual unfit.
g) Epworth Sleepiness Scale (“ESS”) total score of more than 15, indicate severe and
excessive daytime sleepiness thus require further evaluation of sleep disorder
including referral to specialist if necessary.

4.2.3.4 Follow up

i. Ensure that significant clinical findings are appropriately addressed.


ii. In case of uncertainty, consider additional specialist evaluation.
iii. In case of an acute problem causing temporary exclusion, consider periodic review.

4.2.3.5 Notification of Health Assessment Findings

i. Notify employee of results.


ii. Notify the Driver trainer of any impact of any clinical finding on work capability.

4.2.3.6 Medical Form

i. The medical forms below are used by PETRONAS Approved Medical Examiners (AMEs)
panel clinic. Please inform that the test shall be in accordance to PETRONASs’ Drivers
Fitness-to-Work Standard.
ii. For OSH certified medical examiners, the forms need to be brought along and advised
the OSH certified medical examiners to utilize the forms accordingly. The test shall be as
per 5.9.2.
iii. Form 1 and Form 2 shall be filled up by Pilot prior to the visit, or "it may be filled by
doctor or interviewing nurse assistance".

4.2.4 Training Status Form

Transporters Health, Safety and Environment (“HSE”) shall verify that all the recruitment process
steps prior to training have been completed. This include any HR’s employment training. Only then,
Transporter HSE to organize all mandated trainings for Pilot for acceptance as approved Pilot to
carry PCG cargo. For actual form, please refer to Appendix 3.6 - Attachment 1 (Training Status
Form).

4.2.4.1 Training Form

Transporters need to use the forms in RTOG Module 2 (Training) under Appendix 3.6 – Attachment
2 (Pilot Training Minimum Skills/Knowledge Checklist).

4.2.4.2 Letter for Authorization to Drive

A letter of request for ‘authorization to drive’ need to be raised by HSE Transporter to PCG upon
the review that Pilot has completed all the mandated training. HSE transporter shall sign off the
Training Status Form upon his satisfaction. The request shall be made via official letter to S&D
Head of Operation.

Upon approval from PCG, transporters will be authorized to apply for Driver Safety Passport
(“DSP”). The application can be made through [email protected].

Upon all key documentations have been submitted and verified, Pilot will be issued with DSP and
shall carry them when entering designated loading areas. Refer to Appendix 3.8 for the detail
process.

As the issuance of DSP is contracted to a third party, PCG shall ensure the validation of the DSP
issuance to be conducted on periodic basis other than issuer.

4.2.4.3 Endorsement for On Job Training

Applicant will be place under On-Job training under Transporter’s Operation responsibilities after
receiving DSP. It is expected that Pilot will be attached to Pilot’s mentor to cover the following:

i. Loading & Unloading and documentation works at terminal and customer site.
ii. Familiarization and application of transportation operation and safety documentations at
various places.

4.2.4.4 Probationary Period

Pilot will be placed in ‘Probation Pilot database’ with probationary status for a period of not less
than 3 months unless otherwise determined by Law.

During this probationary period, the Pilot should demonstrate safe driving behavior under the
following term:

i. No Incidents (if so Pilot will be counseled and disciplinary matrix will be enforced)
ii. No Traffic Summons (if so Pilot will be counseled and disciplinary matrix will be enforced)
iii. Not suspended by PCG based on OTC/GPS violation record
iv. Not disciplined due to violation in Terminal

By the end of probationary period, Pilot shall be re-evaluated on the defensive driving competency
test to assure Pilot’s knowledge is retained (Appendix 3.9). Letter of confirmation should be issued
if Pilot pass the re-evaluation and notify PCG to remove from the Probation Pilot database’ list.

If not the, probationary period to be extended by another 3 months with second re-evaluation to
be conducted. Transporter is recommended not to confirm the Pilot if failed on second test.

Transporters need to use the forms from PCG Probation Pilot Procedure which can be request from
PCG HSE department.
