This  adoption  agreement  is  made  by  (Rescuers  name)_____________,  which  hereinafter  shall  refer  to  the  same  
entity.  
 
I  agree  to  adopt  the  pet  knows  as__________(described  above)  and  by  doing  so,  I  accept  the  terms  of  this  
agreement  as  set  forth  bellow.    I  understand  and  agree  that  ____________(Rescuers  name)  reserves  the  right  to  
reclaim  the  dog  if  any  of  the  conditions  described  bellow  are  not  met.      
 
________________(Rescuers  Name)  may  also  repossess  the  pet  if  there  is  reason  to  believe  that  the  pet  is  being  
mistreated,  abused  (physically  or  verbally),  neglected,  or  is  in  an  unhealthy  condition.    I  further  understand  that  
________________(Rescuers  Name)  representatives  may  make  a  follow  up  visit,  at  a  mutually  convenient  time,  
to  ascertain  that  all  conditions  continue  to  be  satisfied.    If  I  fail  to  comply  with  the  terms  of  this  agreement,  I  will  
reimburse  ________________(Rescuers  Name)  for  any  costs  incurred  to  reclaim  and  remedy  the  breach  of  
contract  (lawsuits,  etc.).

Please  initial  each  of  the  following  conditions  bellow  to  signify  that  you  have  read  and  agreed  to  each:  
 
______1)      The  dog  will  be  housed  at  my  home  as  a  family  pet,  will  sleep  indoors  at  night  and  shall  have  access  
to  the  residence.    He/She  will  not  be  restricted  to  the  yard  nor  tied  there.    OUR  POLICY  IS  TO  PLACE  
DOGS  AS  FAMILY  PETS  ONLY.    A  lawsuit  will  be  filed  against  any  person  adopting  an  animal  from  us  for  
any  other  purpose(s)  whatsoever,  including  Guard  Dog  Service  or  any  illegal  activities.  

______2)      I  will  provide  veterinary  care  and  treatment  as  necessary  and  agree  to  maintain  recommended  
vaccinations.  I  agree  to  license  my  dog  as  required  by  local  ordinances.  I  will  provide  proper  nutritional  food  and  
fresh  water  will  be  available  at  all  times.  

______3)    A  personalized  identification  tag  with  my  name  and  two  phone  numbers  will  be  worn  all  times.  When  
in  public  areas,  streets,  parks,  etc.,  the  dog  will  be  leashed  at  all  times.  Leashes  will  be  a  maximum  of  six  feet  in  
length,  and  will  be  attached  to  a  martingale-­‐type  collar  or  head  halter.  I  will  not  use  a  retractable  leash.    

______4)      I  understand  that  adopting  a  pit  bull,  pit  bull-­‐type  or  pit  bull  mix  requires  additional  responsibility  to  
promote  positive  perceptions  of  the  breed.  I  understand  my  obligation  to  be  a  Responsible  Owner  by  further  
educating  myself  on  the  special  needs  and  challenges  of  this  breed.  As  a  Responsible  Owner  I  agree  to  
obedience  train,  provide  a  structured  environment  and  establish  a  positive  leadership  role.  

______5)    I  understand  my  responsibility  to  ensure  that  this  pet  is  always  supervised  in  the  presence  of  small  
children  or  other  animals,  as  should  every  dog.  I  also  understand  that  part  of  basic  dog  management  is  to  
carefully  supervise  my  dog  in  new  situations  or  in  any  situation  that  may  be  stressful  to  him/her.  

______6)        IF  FOR  ANY  REASON  I  AM  UNABLE  TO  KEEP  OR  PROVIDE  ADEQUATE  CARE  FOR  THIS  PET,  I  MUST  
RETURN  IT  TO  ____________________.    I  will  not  sell  or  give  this  pet  to  any  other  person,  friend,  relative,  
organization,  medical  laboratory,  or  animal  shelter.  
______7)      If  I  move  to  a  new  location  I  will  notify  ______________________  of  my  new  address  and  phone  
number.  ____________________  MUST  BE  NOTIFIED  IMMEDIATELY  IF  THE  PET  SHOULD  BECOME  LOST  (also  
notify  if  deceased).  

_______8)      To  assist  us  in  continuing  our  rescue  work,  we  request  an  adoption  donation  of  $200.00.  This  
donation  is  immediately  used  to  pay  the  ongoing  costs  of  caring  for  additional  rescue  dogs  including  food,  
veterinary  care,  supplies,  etc.  

There  is  a  two-­‐week  trial  period  that  begins  the  day  you  take  home  your  adopted  dog.  If,  for  some  reason,  you  
are  not  happy  with  your  new  companion  you  may  return  him/her.    
      •   100%  of  the  donation  is  refundable  for  three  days.    
      •   80%  of  the  donation  is  refundable  for  two  weeks.  
The  adoption  donation  covers  only  the  vaccinations  given  to  date.  After  the  trial  period,  I  understand  my  gift  to  
__________________________  is  non-­‐refundable.  
 
 
I  understand  that  ____________________  (Rescuers  Name)  makes  no  guarantees  or  warranties  regarding  the  
health  or  temperament  of  this  dog.  I  promise  and  agree  to  be  solely  responsible  for  this  dog,  and  to  indemnify  
and  hold  harmless  ____________________________  (Rescuers  Name)  from  any  and  all  claims  of  liability  for  the  
conduct  of  this  animal  on  or  after  the  date  of  this  adoption  contract.  This  Release  of  Liability  and  
Indemnification  shall  apply  to  all  known  and  unanticipated  damages  resulting  from  my  adoption,  ownership  or  
control  of  such  dog.  In  addition,  I  agree  to  read  and  sign  the  attached  Voluntary  Release  /  Waiver  of  Liability  
concurrently  with  the  execution  of  this  Agreement.  

I HEREBY ACKNOWLEDGE THAT I HAVE READ AND UNDERSTAND THE ABOVE

From  this  day  forward,  I  hereby  release  ________________(Rescuers  Name)  from  any  liability  for  injury  or  
property  damage  arising  from  any  ownership  of  the  pet.    I  have  read  and  understand  the  terms  of  this  
agreement  and  I  sign  it  freely  and  voluntarily.
