PhilHealth is now providing free copies of mandatory Admission Logbooks for all Level 1 Hospitals. Representatives from accredited Level 1 Hospitals in the National Capital Region can obtain their logbooks from the Accreditation Department in Pasig City. Hospitals in other regions can receive logbooks from their local PhilHealth Regional Offices. The Admission Logbook is considered PhilHealth property and must be used and accessible in accordance with PhilHealth policies.

PhilHealth Admission Logbooks


for Level 1 Hospitals Now Available

We are now providing copies of the mandatory,


PhilHealth-owned Admission Logbooks for all
Level 1 Hospitals for free.
Representatives of accredited Level 1 Hospitals
in the National Capital Region may now get their
logbooks from the Accreditation Department, Room 1202,
12/F Citystate Centre, 709 Shaw Blvd., Pasig City.

Hospitals in the regions may get their logbooks


from our Regional Offices in their areas.

The Admission Logbook is considered PhilHealth


property and shall be used by accredited institutional
health care providers in compliance with PhilHealth
Circular 25, s-2005. It should be accessible anytime to
PhilHealth staffmembers and may be replaced by
PhilHealth anytime.
