Dear Atty. Rex Norman Briones:
I, Ms Maria Clara, 29 years of age, single, (civil status), with residence address at 15A Dahlia St.,
Brgy. Bagong Silang, Laguna, and telephone number 09058713038 and e-mail address at
mariaclara@[Link], am applying for free legal assistance regarding the matter I will be relating.
I fully understand the explanation of LSP Christian Joy Carabbacan, and hereby confirm that:
1. The legal service to be provided by the Legal Aid Clinic - PUP College of Law (LACP) will be
primarily conducted by Law Student Practitioners (LSPs) who are law students enrolled in
Polytechnic University of the Philippines College of Law (PUP COL), under the control and
supervision of the Director and the Supervising Lawyers.
2. The Supervising Lawyers have been designated by the Chief of CLEP pursuant to the authority
accorded under A.M. 19-03-24-SC Revised Law Student Practice or Rule 138-A of the Rules of
Court. The said Rule has been thoroughly explained to me by the LSP.
3. The Chief of CLEP has the sole prerogative to determine if an application will be approved, based
on the rules and regulations of the Clinical Education Program of PUP COL. The financial status of
an applicant is only one of the factors to be considered in approving the application.
4. Once my legal matter is accepted and a determination has been made that I am an indigent client, I
will be accorded with free legal services.
5. In case of litigation, I will pay for any and all expenses incurred therein inclusive of the payment of
docket/filing fees, transcript of stenographic notes, notarization, postage and courier service,
copying, printing, transportation and similar expenses.
6. In the event that a favorable judgment be rendered, or a compromise agreement/settlement be
reached, and an attorney’s fees is awarded, such fees shall accrue to the LACP.
7. Aside from the foregoing, the following are grounds for the termination of the lawyer-client
relationship after formal notice to the client:
a. Failure to fully and truthfully inform the LACP of the client’s economic status;
b. Failure to inform the LACP of the existence of other cases and/or legal issues discussed or
consulted with other lawyers, if any;
c. Failure to inform the LACP of matters which might materially and/or adversely affect the
outcome of the case;
d. Failure to secure and provide the pertinent documents to the LACP, inclusive of the
transcript of stenographic notes;
e. Failure to meet or to discuss with the LSP or Supervising Lawyer when requested by the
LACP;
f. Disclosure of any matter in relation to my case or other cases handled by LACP to media
(radio, TV, social media) unless otherwise authorized in writing by the Director
g. Absence in hearings or proceedings where the client’s attendance is necessary or required;
h. Failure to pay the litigation and out-of-pocket expenses.
8. The applicant shall truthfully reveal, without reservation, all matters related to the case.
I will give my full support to the Law Student Practitioner and the Supervising Lawyer. If
directed, I will personally follow-up the status of the case with the court, tribunal, office, or agency
concerned, and otherwise give my time and efforts in pursuing my case.
I fully understand that the LACP has no obligation to approve my application. I also understand
that LC has no obligation to act upon my application within a fixed period of time. Pending approval, I
accept that the LACP has no responsibility for (1) the lapse of any period to file an appeal; (2) failure to
attend any hearing; or (3) comply with any other deadline or period prescribed by law, the courts, or any
other agency in relation to the matters I referred to LACP.
