Upper Quarter Screen Template

Initial Observation:
Anterior View: Head Position, Shoulder Height, Symmetry of Muscle Mass
Lateral View: Head Position, Shoulder Positions, Thoracic Spine

Cervical AROM:
Flexion (if Full ROM, apply overpressure)
Lateral Flexion (if Full ROM, apply overpressure)
Rotation (if Full ROM, apply overpressure)
Extension (if Full ROM, apply overpressure)

Shoulder AROM:
Flexion
Scratch Test (Internal & External Rotation)

Manual Muscle Test


Cervical Rotation (C2)
Shoulder Elevation (C3-4)
Shoulder Abduction, ER, & IR (C5)
Elbow Flexion (C6)
Elbow Extension (C7)
Wrist Flexion (C7-8)
Wrist Extension (C7-8)
Thumb Extension (C8)
Finger Abduction / Adduction (C8-T1)

Muscle Stretch Reflexes


Biceps (C5)
Brachioradialis (C6)
Triceps (C7)

Sensation
Dermatomes C3-T1
C1 = superior aspect of cranium
C2 = occiput and sides of cranium
C3 = lateral aspect of neck
C4 = superior aspect of shoulder
C5 = deltoid area and lateral forearm
C6 = thumb and lateral forearm
C7 = middle finger (posterior aspect)
C8 = 5th finger and ulnar border of hand
T1 = medial forearm/arm region

Initial Observation:

Thoraco-lumbar AROM:
Flexion (if Full ROM, apply overpressure)
Lateral Flexion (if Full ROM, apply overpressure)
