III. PRESENTING THE PROBLEM

The patient was now admitted at Malita District Hospital ( MDH) and needs
medical assistance including one bag type AB for her blood transfusion and
stainless steel for bone surgery.

IV. CASE BACKGROUND

a. Client's background

Julieta Payan is a thirty-six years old a residents at barangay Pangaleon


Malita Davao Occidental. She is a married woman for almost seventeen years
with the spouse Mr. Roger Payan and a mother of three children. She Belongs to
the Indigenous community which is taga kaulo tribe but spoke fluently in bisaya.

Thus, an exactly December 29, 2022 is a day happened a terrible


accident of Mrs. Payan with bad weather and road slippery and potholed road a
main reason of imbalance control. According to Mr. Payan which is the driver and
husband of Mrs. Julieta Payan they travelled together from Pangaleon to
Poblacion to buy some items for the preparation of new years eve activity. Along
way back home the couple accidentally loss control the motor vehicle and slipped
in the road.

Furthemore, Julieta describe as a shy person who have a long black hair,
big noll eyes with fair skin and average height. During interview she cannot talk
because of her situation that freshly in motor accident cause her dysarthria.

b. Family Background

Currently, the family Payan lived at barangay Pangaleon with a humble


home for almost years. Their place located at remote area under the Panagleon
Sitio that need to travel an hour to reach the center of their barangay. Farming is
the main source of their income with approximately three thousand monthly and
up that it depends on the weather. When, it comes to sanitation and water they
have their own toilet that composed of half concrete and bamboo with floor
level bowl. Their source of water comes from the community water system.

c. Community

Barangay Pangeleon is a placed of Kaulo tribe and a remote area that


surround many threes where fruits and vegetable are abundant. The primary
source of income of the community is farming and construction work because of
a low educational attainment. Teenage pregnancy was very rampant on this area,
the age rate of teenager getting pregnant in this area starting at 14 years old
being the lowest and so on. Due to early pregnancies most teenage mom ended
up as a home maker while the husband do work like farming, laboring,
construction just to provide their daily needs according to Mr. Roger.
V. Assessment

The patient suffered some bruise around her body and a broken bone on her
right leg. The patient was admitted at Malita District Hospital and waiting for the
schedule for her surgery. The patient has a pending hospital bill and the
payment for the stainless steel needed for the operation.

The situation describe as a crisis event of Mrs. Payan and her husband that
need medical and financial assistance cause of motor accident that takes a
temporary disturbance and abilities to dysfunction of a client. The family Payan
decided to sold their small lands and animals to prepare for Julieta’s operation.
Basically they find any ways to provide the needs of her medical problem. The
best method for this situation is the crisis intervention approach which involve a
process for actively influencing the social dysfunction of a person with acute
disequilibrium. This approach is divide into three phases which is assessment,
implementation and termination. Through assessment she can guarantee the
clients is on action for her current situation with initial interview to trace the
subsequent blows that may have aggravated the effects of the initial impact like
trauma for the accident and also help her for medical treatment.

Then, for implementation, this phase is about setting up and working out.
This phase is an action that we should give her an appropriate solution to
alleviate her needs from material arrangement task to psycho-social task.
Lastly,The termination phase, the task should be accomplish accordingly and
build a good relationship with your client.

VI. PLAN OF ACTION


Goal: To assist the client to the service providers to avail financial/medical
assistance that on the 4th week of January 2023 the patient will be able to
receive available blood transfusion and operation.

VII. IMPLEMENTATION/ACTION TAKEN

 The intern was able to conduct daily monitoring at the surgery ward to build
rapport and conduct an intake interview to gather the patient's initial
information.
 The intern sought help to the Registered Social Worker in the agency for
individual counseling in ordered to convinced the clients family for the said
surgery.

 By the end of 4th week, the intern will end the helping process and transfer
the client to the professional personnel for the reaching client's goal and
evaluation for further Social intervention.

VIII. EVALUATION AND RECOMMENDATION

According to the assessment described above,the patients family is


already struggling financially and only makes the minimum salary. The
undersigned therefore strongly advises that he apply for financial assistance in
the amount of 35,000 pesos to cover the cost of the stainless steel required for
for his wife operation.
