Sounded Fire Alarm . Duty officer observed position of fire on Fire Panel. Fire in
Engine room ( Boiler fire ) and immediately inform Master. Muster announced ‘’

Fire in Engine room ( Boiler fire ), crew proceeded with their responsibilities as
per Muster List to Fire Station #0

Communication between Bridge , ECR and Fire Team conducted . Team leader
16:12 reported that all engine department come out from Engine on deck 3 ( Master
station )

16:15 Team leader start count crew – all present

Team leader per command of M aster start to activate CO2 system in engine
16:16

16:20 Team Leader Informed the Bridge that CO2 in progress

16:25 The fire is extinguished . Fire Drill carried out

Note:

1) When re-training on board the ship is required or identified, the Master shall ensure that the crew
members are trained without delay. Records for such retraining shall be maintained in Q19.
2) Re-training required ashore shall further be mentioned in DE-17 (Appraisal Report).
3) The suitability and adequacy of each stated emergency procedure shall be reviewed by the Master
while conducting the drills and suggestions for revisions shall be recorded. Form Q 8– Change
Proposal Form shall also be filled up to initiate effecting of these changes.
