Is the type, condition and integrity of the scaffold material
acceptable?
Are reinforced (double) gantries installed for scaffolds exceeding a
height of 10m?
Are single gantries installed for scaffolds exceeding a height of 4m?
Are all gantries erected off-center from the material handling line,
where possible?
Is only one steel board removed from gantries through which
material can be passed?
Is a dedicated chargehand supervising/directing the handling line
for dismantling of scaffold ≥10m?
Are barriers erected around the area to prevent persons from
entering unintentionally?
Are signage displayed to indicate work in progress above or caution
potential drop zone?
Are people above 2 meters adhering to 100% tie-off rule?
Is the harness tie-off point safe?
Do persons tie-off above shoulder height when working >2m?
Did fall arrest equipment undergo the required inspections?
Are safety harnesses in good condition? (no damage, etc.)
PPE suitable and in good condition? (Gloves, coveralls, boots, etc.)
Are hard hats secured with a chin strap while working at heights?
Are tool lanyards attached to hand tools to prevent them from falling
to the ground?
Do scaffold builders have pouches to keep their tools secure so it
does not fall from heights?
Is scaffold material stacked neatly out of walkways/work areas
eliminating tripping hazards?
Are temporary laydown areas properly demarcated (made visible)
by use of barricading like high visibility safety tape, netting, etc.?
Are the edges of temporary scaffold laydown on elevated plant
areas protected to prevent material from falling?
Is a red tag attached to the scaffold during erection, dismantling or
modification activities?
Are barricading scaffolds tagged? (For high pressure blasting or
jetting, excavations, pedestrian channels, laydown areas)

Is an approved risk assessment/LARA available for scaffold
activities in poor weather conditions?
Are alternative measures implemented where scaffolding obstructs
emergency equipment?
Is the twist and turn practice adhered to during material handling?
Scaffold builders are not standing more than 2m apart when passing
material?
No persons are standing next to the gantry, in the drop zone (line of
fire)?
Scaffold builders are placed according to experience, least
experienced at the bottom?
Is material handled on staircases along the flow of the staircase and
not through handrails?
Scaffolds are not being supported by equipment, cable racks, drains
or process lines?
Is the condition of scaffold material trollies acceptable?
Are scaffold material trolleys handled by the correct number of
employees? (4 people when loaded)
Are stop blocks in place against the wheels of scaffold material
trollies when required?
Unauthorized persons stay outside barricaded areas?
Scaffolding material not thrown to ground against each other?
Safe route considered when moving material trolley?

ONLY Competent and Authorized Scaffold Builders can erect or modify and dismantle scaffolds and
ONLY Competent and valid certified inspectors can approve scaffolds.
Evidence of competency, safe condition, and validity to be attached as support documents to RA and JSA.
