PHYSICAL EXAMINATION STATEMENT:
I __________________ have examined _________________ and found him/her physically fit for
playing.
CONTRACT DETAILS:
Player and player’s parents / guardian will be responsible for the punctuality and regularity.
Player is not allowed to miss the training session without prior permission of the management/
coach.
Player missing training session without permission will not be allowed to join next training
session.
Missing three consecutive training sessions in a month without permission will result the
termination of the contract.
Player is not allowed to join any other football/futsal/feet ball club or academy during the
contract period with TWK.
Football training equipment will be provided by the TWK.
Players and their parents/ guardians will conduct themselves in accordance with the TWK
rules at all times or will be subject to disciplinary action.
Violating the discipline/ rules of the TWK will result in the termination of the contract.
The total cost for the entire one year program Rs. 200,000/- only per player. (Non refundable)
TWK has the right to change the terms and condition time to time.
