Case Study: Improving Services at the Ministry of Health Hospital
Background
A Ministry of Health hospital located in a provincial capital serves a strategic
role as the main referral center for the region. However, a recent evaluation
identified several major issues as follows:
1. Emergency Department (ED) Performance:
○ Patients in the ED often have to wait more than an hour before
receiving medical attention.
○ Coordination between medical and non-medical teams
(administration, pharmacy) is suboptimal.
2. Referral Management:
○ Many referral patients from primary healthcare facilities lack
complete documents or data, causing delays in treatment.
○ The online referral system (SISRUTE) has not been optimally utilized
by the hospital.
3. Operational Efficiency:
○ Average patient wait times for radiology and laboratory procedures
range between 2-4 hours.
○ Some advanced medical equipment is underutilized due to a
shortage of certified operators.
4. Data Quality and Reporting:
○ Reporting of health service data to the central authority (Directorate
General of Health Services) is often delayed and incomplete.
○ Digital medical records are not yet fully integrated across hospital
units.
As a PASSKAS candidate, you are required to provide strategic solutions to
these issues by answering the following questions:
1. Problem Analysis
● Identify the root causes affecting each of the aspects mentioned
above.
● Explain the impact of these issues on the hospital's overall
performance.
2. Solution Plan
Develop a work plan that includes the following steps:
● Improving ED Services: How can response times and coordination
in the Emergency Department be enhanced?
● Optimizing the Referral System: Strategies to ensure the
completeness of referral patient data and maximize the use of
SISRUTE.
● Operational Efficiency: Recommendations to reduce patient wait
times for radiology and laboratory services and improve the utilization
of medical equipment.
● Data Integration: How to develop and integrate a digital medical
record system to ensure faster and more accurate reporting to the
central authority?
3. 3. Stakeholder Engagement (Stakeholder Mapping)
● Identify internal parties (hospital management, medical team, pharmacy, IT) and
external parties (Health Office, Ministry of Health, BPJS Health) that need to be
involved.
● Develop coordination strategies to ensure all stakeholders support the implementation
of the solutions.
4. 4. Monitoring and Evaluation
● Define Key Performance Indicators (KPIs) to measure the success of the solutions.
● Describe how you will conduct regular monitoring and evaluation to ensure the
program runs according to plan.
5. 5. Expected Outcomes
● Outline the concrete results to be achieved within 6 months and 1 year, along with a
timeline and milestones.
● Develop strategies to ensure the sustainability of the program after the initial
implementation.
