ACCIDENT REPORT TIMINGS
1645lt E/T Palma and Electrician left the elevator together to take dinner.
1700 lt E/T Palma noticed that electrician is not yet present at dinner table, thinking that EO was still at
changing room, so he finished his dinner.
1740lt after E/T Palma finished his meal, he started looking for Electrician but he cannot find him anywhere
and as he pass by chief Engineer who is taking his dinner too, he told him that the EO is missing, Chief engineer
knowing that the last known location of Electrician was on the elevator shaft, he ask to immediately check the
last position where the elevator was stopped. The elevator was on deck “A”.
1745lt they open the elevator door on deck “B” using the emergency key to view the top of shaft of elevator
and there he was found caught between the beam and the counter weight.
1747lt – Chief engineer informed the Master at once on the situation.
1748lt – Master call all the crew to help pull out the electrician from the elevator shaft.
1750lt- Master reported the incident to DPA, call the nearest authorities for emergency medical assistance,
informed charterer.
1815lt- EO body was retrieved from the elevator shaft. Checked for vital signs, it was confirmed by 2/O that EO
has no pulse, the physical appearance is black and face and right arm are heavily swollen, blood and saliva
coming out from his mouth.
1820lt - EO transferred to hospital and continue monitoring situation while waiting for medical assistance.
2222lt- Master received instruction from charterer agent (Monson China) to sail northwest of the Chaolian Dao
Light House to meet rescue vessel “BEI HAI JIU 113” who will attend/pick up the EO.
2300lt – Commenced heave up anchor.
2324lt – Anchors aweigh.
2325lt - Vessel started sailing northwest of Chaolian Dao Light House.
2350lt – Rescue vessel “BEI HAI JIU 113” advised to drop anchor south of Chaolian Doa Light house.
06th Apr. 2017- Anchored South of Chaolian Dao Light House
0136lt – Dropped stbd anchor in position lat. 35 48.0N, long 120 52.5E or brg 180 deg distance 5.42 nm.
0210lt – Rescue vessel “BEI HAI JIU 113” alongside port side.
0216lt – 5 personnel came on board (2 medical personnel, 1 camera man, 2 assistants).
0220lt – Medical personnel checked the condition of electrician officer.
0245lt – 5 personnel disembarked/EO body remained on board/according to Captain of the rescue vessel they
will wait for instructions from their main office.
0250lt – Rescue vessel cast off and remains nearby.
0323lt- Received instruction from DPA to preserve the body of EO following the Ship’s Medical Guide and
moved it to one of the reefer chambers. Master ordered the crew to vacant the fish room and prepared for
occupancy EO’s body.
0400lt – Rescue vessel “BEI HAI JIU 113” advised (via VHF-08) they will return to port as instructed by their
main office.
0405lt – due to body of EO did not discharge, master inform owner’s agent for additional instruction.
0410lt – Master requested from VTS on VHF-08 if vessel can keep present anchor position until EO body is
discharge. They reply stand-by
0739lt- Confirmed to DPA that the fish room is ready for occupancy of the remains, body placed into the body
bag, ready for transport.
0743lt - DPA sent confirmation that remains of EO can be moved into the fish chamber.
0921lt- Replied to DPA’s confirmation message and start moving the body.
1100lt - EO’s remains was in place at the fish chamber and informed all concerned parties.
