PARENTAL CONSENT FOR STUDENT INTERNSHIP

I hereby willingly and voluntarily give consent to Althea Rustian G. Villasis a

Bachelor of Technology Livelihood Education Major in Information and

Communication Technology student from College of Education, Taytay Campus to

undergo 360 hours of face to face student internship at Host Training Establishment

from inclusive dates.

I have considered the benefits that my child will derive from undergoing face to

face internship, with the understanding that due care and precaution will be observed to

ensure the comfort and safety of the intern/student to this activity and that I shall not

hold the parties responsible for any untoward incident that may happen beyond their

control.
