Observed gnats near floor drains.
Observed gnats inside of container of butter left out overnight.

Facility was last exterminated by Orkin on 3/17/25. Reported stated gaps in front/back doors and
cleanliness issues.

228.186(k)(1-4) (k) Controlling pests. The presence of insects, rodents, and other pests shall be control to
eliminate their presence on the premises by:
(1) routinely inspecting incoming shipments of food and supplies;
(2) routinely inspecting the premises for evidence of pests;
(3) using methods, if pests are found, such as trapping devices or other means of pest control as
specified under §228.204(b) and§228.208(b) and (c) of this title; and
(4) eliminating harborage conditions.

OUT 36. Wiping Cloths; properly used and stored

Observed wiping towels not in buckets.

Containers of chemical sanitizing solutions specified in paragraph (2)(A) of this subsection in which wet
wiping cloths are held between uses shall be stored off the floor and used in a manner that prevents
contamination of food, equipment, utensils, linens, single-service, or single-use articles. Cloths in-use for
wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizer solution
at a correct concentration. Cloths in-use for wiping food spills from tableware and carry-out containers
that occur as food is being served shall be maintained dry and used for no other purpose. Dry wiping
cloths and the chemical sanitizing solutions in which wet wiping cloths are held between uses shall be
free of food debris and visible soil.

(1) Equipment food-contact surfaces and utensils shall be clean to sight and touch.
(2) The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease
deposits and other soil accumulations.
(3) Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food
residue, and other debris.

228.186(d) Cleaning ventilation systems, nuisance and discharge prohibition. 228.104(1) Intake and
exhaust air ducts shall be cleaned, and filters changed so they are not a source of contamination by dust,
dirt, and other materials.

R 44. Garbage and Refuse properly disposed; facilities maintained

Observed dumpster needs to be replaced by next inspection.

228.152(f)(1-2),(o) §228.152. Refuse, Recyclables, and Returnables, Facilities on the Premises. (f) Outside
receptacles.
(1) Receptacles and waste handling units for refuse, recyclables, and returnables used with materials
containing food residue and used outside the food establishment shall be designed and constructed to
have tight-fitting lids, doors, or covers.
(2) Receptacles and waste handling units for refuse and recyclables such as an on-site compactor shall
be installed so that accumulation of debris and insect and rodent attraction and harborage are minimized
and effective cleaning is facilitated around and, if the unit is not installed flush with the base pad, under
the unit. Using drain plugs. Drains in receptacles and waste handling units for refuse, recyclables, and
returnables shall have drain plugs in place.

NOTE: DUMPSTER LIDS SHALL REMAIN CLOSED WHEN NOT IN USE.

228.186. Premises, Buildings, Systems, Rooms, Fixtures, Equipment, Devices, and Materials. (a)
Repairing. The physical facilities shall be maintained in good repair. (b) Cleaning, frequency and
restrictions. (1) The physical facilities shall be cleaned as often as necessary to keep them clean. (2)
Except for cleaning that is necessary due to a spill or other accident cleaning shall be done during
periods when the least amount of food is exposed such as after closing.

(f) Drying mops. After use, mops shall be placed in a position that allows them to air-dry without soiling
walls, equipment, or supplies.

228.174(e)(3)(f) Exterior walls and roofs, protective barrier. Perimeter walls and roofs of a food
establishment shall effectively protect the establishment from the weather and the entry of insects ,
rodents, and other animals.
