MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This MEMORANDUM OF AGREEMENT made and executed between
CAMBERWELL TECHNICAL EDUCATION AND ASSESSMENT CENTER INC.,
represented herein by its Chairman of the Board of Trustees (BOT), GRACE M.
RODRIGUEZ is a Technical Vocational Institution duly registered and existing under
and by virtue of the laws of the Republic of the Philippines with registered address
at Magsaysay Nagulian Isabela, hereinafter referred to as FIRST PARTY;
-and-
CAMBERWELL CONSTRUCTION AND SUPPLIES represented herein by
its Department Head, Dir. ANTHONY PETTE GO RODRIGUEZ a private
construction company organized and existing under and by virtue of the laws of the
Republic of the Philippines with official address at Naguilian , Isabela hereinafter
referred to as SECOND PARTY;
WITNESSETH THAT:
WHEREAS, the FIRST PARTY offers, provides and conducts technical and
vocational courses such as; HEAVY EQUIPMENT OPERATION NC II, HEO
WHEEL LOADER NC II, HEO HYDDRAULIC EXCAVATOR NC II, HEO DRIVING
NC II, HEO BACKHOE LOADER NC II, HEO BULLDOZER NC II, HEO CRAWLER
CRANE NC II, HEO TRUCK-MOUNTED CRANE NC II, HEO ROAD ROLLER NC
II, HEO TRANSIT MIXER NC II, HEO ROUGH-TERRAINE CRANE NC II AND
HEO FORKLIFT NC II to interested student-trainees under the supervision of the
TECHNICAL EDUCATION SKILLS AND DEVELOPMENT AUTHORITY (TESDA);
WHEREAS, the SECOND PARTY has a direct administration and
supervision over the duties and responsibilities of the Ilagan , City Medicat Center ,
Isabela headed by Dr. _________, MD;
WHEREAS, the FIRST PARTY intends to avail of the medical services of the
SECOND PARTY to which the latter agreed to render medical examination/health
services to all student/trainees who will undergo training and assessment at Angels
Touch , INC.;
NOW, THEREFORE, for and in consideration of the foregoing premises, the
parties herein agree and stipulate the following:
1. That the FIRST PARTY shall ensure safety and protection of all its
students/trainees who are undergoing training and assessment within its
facility/vicinity;
2. That in cases of unavoidable and inevitable injuries or accident in the training
and assessment facility, the SECOND PARTY shall accommodate and
render medical examination/health services to student/trainees who suffered
aforesaid injuries either caused by co-student, staff or third person or injuries
 by accident while on training and assessment or while the student/trainee is
within the vicinity;
3. The FIRST PARTY shall shoulder the medical expenses (medicines/x-ray/ct-
scan etc) incurred by the student/trainee, if there be any;
4. The term of this agreement is 5 year/s as agreed upon by the parties, which
shall commence upon execution of this deed and shall end on _________
subject to renewal upon the mutual option of both parties;
5. The parties hereto have executed this deed voluntarily and any violation or
willful violations of any of the terms and conditions herein shall cause the
rescission of the contract with notice.
