Important Note
1. Every Pharmacist who is registered under Uttar Pradesh Pharmacy Council should immediately Apply
online, if any change is required in his/her registered Address/Qualification/Name etc. He/she will be bound
to answer all the queries raised by Uttar Pradesh Pharmacy Council, at the earliest.
2. All persons who are registered as Pharmacist under Uttar Pradesh Pharmacy Council are legally qualified
for the Practice of Pharmacy.
3. Every first time registered Pharmacist should get his/her Registration renewed before the first day of April,
immediately following the registration, according to the provisions of Section 34 of the Pharmacy Act, 1948.
After the first Renewal of Registration, Pharmacist will have to get their registration renewed at every 05
years.

Verify Certificate through QR Code


Open QR Code Scanner/Camera of your Smartphone/Tablet or other compatible device, and scan
the QR Code available on the Certificate. Thereafter, a link will appear on the screen of your device.
Click on that link, after which you will be redirected to the Official Portal of Council from where you
can verify the details of the Registered Pharmacist.
