“weAcess”; and ACIC FILE UPLOADING
b) Designate authorized signatory to enroll and designate authorized user/s in the
LandBank weAccess .
As Secretary /Head of the the BARANGAY FAUSTINO SK authorizing Hon, John Paul U.
Gamido, SK Chairperson of Brgy. Faustino / Department of the following:
a) To execute/ sign all document necessary to Subscribe to LBP’s “weAceess”.
b) To designate / authorized personnel to act as rtw “maker or” authorizer” for the
company/ access to LBP,s weAccess banking facility; and
c) To amend add/ delete enrolled deposit account/s as source and or destination account/s
access rights the weAccess modules, authorization rules and or any information on
original and succeeding enrolment.
This shall, as well, be an addendum to signatories of submitted resolution for account
opening.
Attached is the agency/ office enrolment form and other document supporting our
enrollment in your LANDBANK weAccess.

RESOLUTION AUTHORIZING SK CHAIRPERSON [Link] PAUL U. GAMIDO,
AND SK TREASURER MR. JERICHO M. DUCSA TO ENROLL IN THE LANBANK
INSTITUTIONAL INTERNET BANKING "weAccess"
WHEREAS, on May, 2025, the SK Council of Barangay Faustino has approved
Resolution No. 42-2024 authorizing SK Chairperson Hon. John Paul [Link], And Sk
Treasurer Mr. Jericho M. Ducsa to the following:
1. Subscribe to the Landbank of the Philippines (LBP) internet banking facility known as
"weAccess"; ADVICE AND BANK STATEMENT and
2. Designate authorized signatory to enroll and designate authorized user/s in the Landbank
weAccess.
NOW, THEREFORE, as it is hereby Resolved, authorizing SK Chairperson [Link] Paul U.
Gamido, And Sk Treasurer Mr. Jericho M. Ducsa to:
Execute/sign all documents necessary to subscribe to LBP's weAccess”;
1. Designate/authorize personnel to act as “maker” and "authorizer” for the Sangguniang
Kabataan of Barangay Faustino
2. Amend/add/delete enrolled deposit account/s source and/or destination account/s, access
rights to the weAccess modules, authorization rules and/or any information on the
original and succeeding enrollments.
3. And that the same is hereby confirmed and ratified;
RESOLVED FINALLY, to furnish a copy of this resolution to the LBP Cauayan City Branch,
Office of the DILG, and the Office of City Accountant for their information and approval.
