Re: The use of a Man-Cage with defective sling and without a Rigger

This letter serves as a formal disciplinary warning regarding your failure to adhere
to the Company's health and safety policies and procedures. On 21th February
2025, you were found using a man-cage with a defective sling and without a
qualified Rigger present

Our investigation revealed that you failed to follow established safety protocols,
which is a serious breach of our Company's code of conduct. As you are aware,
maintaining a safe working environment is of utmost importance, and it is every
employee's responsibility to adhere to our health and safety policies.

Although our disciplinary code suggests that a final written warning should be
issued for such an offense, considering that this is your first offense and you have
not been disciplined before, we have decided to issue a first written warning
instead.

Please note that any future incidents or failure to comply with health and safety
policies will result in more severe disciplinary action, up to and including
termination.

You are required to:


1. Familiarize yourself with the Company's health and safety policies and
procedures.
2. Attend a refresher training session on health and safety protocols.
3. Acknowledge receipt of this disciplinary warning and confirm your understanding
of the expected improvements.

Please sign and return the acknowledgement form attached to this letter by COB 14
March 2025
