a. Passport, PAN card, Driving license, voter id card, ration card ,any other govt
card with photo, attested proof from bank as identity proof- any one of these
b. Passport, Last 3 months electricity, phone bill, bank passbook copy, attested
copy of bank statement, Voter ID card ( not more than 2 yrs old) – in case
staying with parents, the bills in name of parents can be submitted along with
declaration form the parent – any one of these
4. Date of birth certificate: if PAN, Driving Licence, Election ID or Passport is submitted
this is not required separately
5. Self attested copies of educational qualification. original to be showed at the time of
